How to get started.
The decision to select a skilled nursing care center is often made during a time of illness or
stress, creating heightened family and caregiver emotions, as well as those of the individual in
need of greater care and support.
The discussion frequently begins with a primary care or referring physician after a change in health condition or acute illness requiring hospitalization. We encourage families and caregivers to thoroughly explore all care and support options available within their communities, and to ask questions until they feel comfortable enough to make an informed decision.
Those involved in the decision-making process must feel confident that the skilled nursing care center selected provides for not only their loved one's medical needs, but their social needs as well.
